Key Responsibilities:

As a Care Navigator Apprentice, you will be an essential part of the administrative team, helping patients access the right care and support.

Process and effectively signpost patients to the appropriate healthcare professional based on their presenting condition.
Answer incoming phone calls, transfer calls, or address callers’ requests appropriately.
Manage patient requests for appointments and repeat prescriptions.
Initiate and respond to requests from patients, team members, and external agencies.
Enter read-code data into the SYSTM1 system.
Maintain and organise patient records and documentation, including scanning and attaching documents to patient files.
Assist with system searches, managing repeat prescriptions, and contacting patients for annual reviews or clinical follow-ups.
Ensure that all incoming mail is actioned and contribute to practice audits when directed.
Provide support during staff absences and maintain a clean and organised working area.

Skills & Knowledge:

North Beverley Medical Centre is looking for a friendly, enthusiastic, and reliable individual who is eager to learn and contribute to their healthcare team.

Strong communication skills with a positive phone manner.
Ability to work well in a small team and build good relationships with patients and staff.
A strong commitment to confidentiality and maintaining patient trust.
Good IT skills, including proficiency in Word and Excel.
A flexible approach to working hours, with the ability to cover sickness and annual leave as needed.
GCSE grade 4 or above in English and Maths (or equivalent).

The successful candidate will work towards a Level 3 Business Administration Apprenticeship with the appointed provider, HBTC (www.hbtc.co.uk). An Apprenticeship allows you to acquire valuable skills and experience in your chosen sector, which improves your CV and employability when you've finished training. You'll learn in the work environment, developing the practical skills and understanding you need to excel in your role. This opportunity could also lead to permanent employment and progression opportunities.
